Submission Forms 
It's My Story Too
Thank you for your interest in our project. We welcome your participation and that of other family members.

All submissions need to be written by the sibling. There is no minimum or maximum length. There is no required style. Narratives, first or third person, poems, artwork, etc are all welcomed. Editing is not required. We will fix spelling errors, only to the extent necessary to make the entry legible to others.

Attached you will find two forms.

Form #1 is a data sheet for information on your missing person. On the data sheet, you have the option to be published anonymously. Please carefully consider the option that you choose. We believe that connecting the sibling’s story to the missing person provides a stronger impact to the reader. However, we do not want this opportunity to share to add any additional trauma for the sibling. Some siblings may also find themselves with very strong emotions that may not be the most appropriate to share in direct conjunction with the missing person’s story. Again, please give this some thought and discussion. It is important that you make the choice that is best for your family.

At this time, we can only accept submissions linked to missing persons currently or formerly listed with the National Center for Missing & Exploited Children, the National Center for Missing Adults, or a law enforcement agency that is willing to verify the information.

Form #2 is a permission form for publication. Both the person writing the submission and the parent/guardian must sign the permission form.

Forms should be returned to the address and/or email on the permission for publication form.

Again, we appreciate your willingness to participate in our project. We hope it will be as beneficial to your family as it has been to ours.
